Thank
you for your interest in becoming a student at St. Martin
de Porres High School. Please follow the steps below
to begin your journey SMDP.
1.)
Please download and print our official
application (If you can't open the PDF
copy of the application, please use the links at the
bottom of the page which will let you open the different
sections of the application in Microsoft Word).
2.) Ask a teacher, pastor, and personal contact to fill
out your recommendations. Your recommendations should
be sent directly to SMdP by your teacher/pastor/and
personal contact.
3.) Fill out the application. Please note that you will
need the following materials.
Most
Recent 8th Grade Report Card
School Record or Report Cards from 6, 7 and 8th Grades
Copy of Birth Certificate
Copy of Social Security Card
Results of St. Martin de Porres HS Skills Test
PSAS Form Completed
4.)
Fill out the Request for Records authorization form.
5.) Mail or deliver your authorization form and application
to SMDP by May 15.
